It is certainly possible. When Neverwinter Nights was released, some batches
of CDs were low quality and had the tendency to break apart in faster drives.
Thode disks could be recognized by a missing reinforcement ring near the
center of the disk.
There are stories about disks breaking at completely bad moments, hurting
people who were about to remove the disk from the drive. You _should_ be safe
from this if you're using one of those older caddy-loaded drives (disks can
still break, but those caddies are relatively sturdy and all drives I know
using those things are running at sane speeds <=32x and are much better
balanced than todays cheapbottom drives).
Rule of thumb: if a disk starts to crack at the center, stop using it.
