Do you have all of the following modules loaded?
- usb-storage
- sr_mod
- sd_mod

Looks like you have the "usb-storage" module installed, but are missing
one (or both) of the sr_mod and sd_mod.

It will be listed in the dmesg output and should be one of the /dev/sd?
