Sounds like one of the modules in your /etc/pam.d/exim does not actually
exist. Often, the debian-provided examples include pam_pwdb.so even
though there _is_ no pam_pwdb.so AFAIK. verify that your pam file only
uses pam modules that actually exist under /lib/security 

Of course, that is just a guess :)
