This should work. Add a source line to /etc/apt/sources.list, like:
deb-src ftp://ftp.nl.uu.net/debian stable main contrib non-free

Then, as #root and $user:

# apt-get update
# apt-get install libncurses5-dev mime-support gcc dpkg-dev fakeroot
pine4-src pine4-diffs

$ cd /tmp
$ dpkg-source -x /usr/src/pine4/pine4_4.21-1.dsc
$ cd pine4-4.21
$ fakeroot debian/rules binary
$ cd ..
$ rm -r pine4-4.21

# mv /tmp/*.deb /somewhere/safe
# dpkg -i /somewhere/safe/*.deb
