What an embarassing question. The answer is
a) I'm running mutt in rxvt.
b) I suddenly remember that the last time it worked was when I was still
using gnome-terminal, which I dumped along with the rest of Gnome
when I switched to Blackbox (I'd switched to WindowMaker first, but
was still using gnome-terminal.)
c) I just reinstalled gnome-terminal and verified that the feature works
great: put the cursor over a hyperlink in an open gnome-terminal and
it underlines, ready to be clicked.
And my second question is obviously one for the Mozilla wish list, I
want to be able to click a mailto: link and have it run the shell script
or application of my choice with the mailto: link value passed in as the
$1 parameter.
In my case I'd have it launch the xterminal of my preferred flavor
with mutt running ready to compose a message and the To: field filled in...
