I don't know anything 'bout JADE or DocBook/XML ... but this error message ...
*uhm* .. the only underscore I can see in your example is in this line:
<!ENTITY cap_intro SYSTEM "cap_intro.xml">
have you tried deleting it? E.g. like this:
<!ENTITY capIntro SYSTEM "cap_intro.xml">
or even
<!ENTITY capIntro SYSTEM "capIntro.xml">
???
