Is this an ISA or a PCI card?  If a PCI, you could try "cat /proc/pci"
which will tell you what PCI hardware you've got installed.  You may
be able to distinguish a modem manufacturer (eg Lucent or something).
Also check out www.linmodems.org or try a search with google with the
name of your modem
