How are you configuring X?

My own preference is to use (for XFree86 3.3.x) XF86Setup, which has a
graphical configuration tool under which you can select options for
various settings, including mouse (with keyboard accellerators), until
something works.

Highly recommended, if appropriate.  Not sure what the equivalent tool
for XFree86 v4 is.
