I don't understand what you mean by "The following is the pppconfig:", but
it appears that the instructions in pppconfig are not clear enough.  You
have appended the phone number, username, etc to the place holders instead
of replacing the placeholders with the proper string.  "replace_with..."
means exactly what it says: backspace over the placeholder and type in the
appropriate string in its place.
