One reason that I need it to be one file is that I want to print all the 
document (book actually), and have the printing look nicely. Having to do it 
interactively seems to me too time consuming, and the result will not be good 
enough for my taste.

BTW: I have tried htmldoc in a naive way (without any customizations) and the 
result was poor: although I actually got one file, the order of the sections 
was bad. For example, appendix A turned on in the middle of the first chapter.
