I use fvwm2 and have this:

Exec   xsetroot -cursor ~/X/custom.cursor1.xbm ~/X/custom.cursor1.xbm

in the file:

~/.fvwm2/post.hook

That file looks like this btw:

EdgeScroll 100 100
EdgeResistance 100 100
Exec 	xsetroot -cursor ~/X/custom.cursor1.xbm ~/X/custom.cursor1.xbm
Exec	xv -root ~/X/background.gif -quit
