There are a few Apache directives that affect the .htaccess file.  I could
find:
  AccessFileName
  AllowOverride
Docs for these can be found at:
  http://www.apache.org/docs/mod/directives.html

You probably have one of these set wrong (more likely, the default isn't
what you wanted).
