Your ISP wants to see something more than just username and password. Your
chat is exiting after sending the password and letting pppd start sending
packets. The ISP is echoing those packets while it waits for the
appropriate input. Finally pppd gives up.
Dial in to the isp with minicom and work through the login (taking notes)
until you start getting garbage: that's ppp starting on the ISP end. Then
quit minicom, run pppconfig, and refer to your notes. You will probably
need to put something in "Other Strings".
