I had the same problems. I made the following modifications to my
modules configuration :

In /etc/conf.modules, I added the following lines :

# SB AWE64 PnP
alias char-major-14 sb
post-install sb modprobe "adlib_card"
options sb io=0x220 irq=5 dma=1 dma16=5 mpu_io=0x330
options adlib_card io=0x388

(check the values for io, irq, dma ...)

In /etc/modules, I added :
sb
awe_wave

Now, everything works fine.
