both.  it makes a .deb package of your customized kernel.

i ran:

make-kpkg -revision custom.1.0 kernel_image

from the directory /usr/src/linux, and it put the file:

kernel-image-2.0.27_custom.1.0_i386.deb

into /usr/src

make-kpkg is easier to use and easier to keep track of changes once you
figure out how to use it right.
