I've contacted upstream (Heiko Oberdiek) about this and got an reply
quite fast. The answer in short:

1. the hyperref manual manual.pdf is in reality not the complete
manual, but just a part of it. Some other parts of it are
options.tex, paper.pdf and slides.pdf. These documents theoretically
have to be merged into the manual. Unfortunately Heiko seems actually
to be the only one who writes hyperref and he has a growing TODO
list. So he keeps on coding instead of writing documentation.
2. The source of manual.pdf is not hyperref.dtx. LaTeXing that would
give you the doku of the code, but not the manual. Renaming the
hyperref-manual to hyperref.pdf would give us a filename clash.

He suggested a solution specific to Debian and we'll think about
that. For the time being I tag that bug upstream and wontfix.
