I'm quite sure I have no idea which package this bug is in, but I'll trust
that you'll know better than I, if it is indeed not a docbook-utils bug.

I am unable to generate pdfs w/ docbook2pdf - i noticed this when one of my 
packages began to FTBFS.  The sgml doc that failed was rather large, but I've
reproduced w/ the following (the fact that its a note is arbitrary):
