Ah, I completely misunderstood the report.  I guessed
that context which caused problem was not generated
by default but I noticed it was default one at last.

I checked my log and it was completely same as the
log you attached in the first report until the error
happened.

So modifications I suggested to you are not necessary
and please remove all modifications I suggested (sorry).

Because I have not encountered any problem so there should
be another problem in your system.

Do you have any other TeX system or do you set any TeX
related environment variables etc. ...??
