   * What led up to the situation?

A wheezy to jessie upgrade, then a dist-upgrade.

   * What exactly did you do (or not do) that was effective (or
     ineffective)?

During the dist-upgrade the GUI failed, then CTRL+ALT+Fn to console, as I
guess (?) output hung in GUI console. Needed reboot and dpkg --configure -a
to complete on reboot in a no GUI console. Ok then system up.
PROBLEMS:
/etc/fstab wrong, it got /home and swap wrong, leading to wait on boot and no
swap.
Package virtualbox-dkms not included as per before upgrade.

   * What was the outcome of this action?

A few errors I had to manually fix, but now fixed by a bit of work.

   * What outcome did you expect instead?

A seamless update.

Dear reporter,

Thanks for taking the time long ago to submit your upgrade report. I'm
closing these reports now because the Debian releases they were reported
against have reached their end-of-life (some long ago).

Unfortunately it's possible that the report I'm now closing may still
have relevant information for the current release (bullseye). If you
believe that's the case, don't hesitate to reopen the bug, retitle it
and provide further information and it will be seen during the current
freeze period of Debian.
