Bug#651357: ssh often dies with "Disconnecting: Received open failure for non-opening channel 3."

Package: openssh-client
Version: 1:5.9p1-2
Severity: normal

I use a jumphost to connect to remote servers, something like:

ssh $jumphost
ssh -o ProxyCommand=ssh -q -a -x -W %h:%p $jumphost root@$target

Often the main connection dies with this message:

Disconnecting: Received open failure for non-opening channel 3.

This is not deterministic, so I cannot provide instructions to reproduce
this. It /may/ be triggered by sustained traffic in the tunneled
connection at the beginning of the session.

I use "ControlMaster yes" in ~/.ssh/config and key authentication.

The jumphost runs Ubuntu's 1:5.3p1-3ubuntu6.
