Bug#253743: /usr/bin/scp: scp foiled when input file grows during transfer

If data is appended to a file while it's already being scp'ed to another
location (e.g. because it's being transferred in from another host on a
faster connection), scp appears to ignore the part that was added after
startup. The progress counter reaches 100%, which is 100% of the
partial file size, and stops there.
