This could be the same problem that I experienced on HPPA driver not so 
long ago:

In my case scsi_wait_scan.ko didn't wait for all SCSI disks to be 
discovered, debian initramfs moved on prematurely and failed miserably. 
Recompiling the kernel without CONFIG_SCSI_SCAN_ASYNC fixed the problem 
for me.

What SCSI controller / driver have you got? The output looks very 
similar to my problematic sym53c8xx. It could be a bug in that driver...
