All these issues seem to be caused by the pulseaudio default value "flat-volumes = yes" (see /etc/pulse/daemon.conf). I strongly suggest to change it to flat-volumes = no by default (even some upstream maintainers seem to agree [1]), this is already the default in ubuntu and arch.

For the users affected by this, please set flat-volumes = no in /etc/pulse/daemon.conf or in the ~/.config/pulse/daemon.conf file.
