I've had this problem too. It is not limited to Firefox. You can use alt+tab to get back to your window. I remember reading a massive kde bug report about it early last year. Something to do with apps unexpectedly changing something about their own window state while in the background which causes the panel to lose track of it.. If you can come up with steps to reliably reproduce the problem, it would be greatly appreciated by upstream as they have tried to fix this numerous times in the past.

from time to time, the task manager from KDE looses the iceweasel
windows, i.e. they're not listed anymore in the task bar. I didn't find
any specific pattern for this to appear but it appears quite regularly,
and (till now) for no other application, not even icedove.
