If I have an anchor called "something" in an html file, then
index.html#SOMETHING doesn't find it, so it should be case insensitive.

Please read the following text from the same document that you ( the
OP ) quoted ( only a few lines below, really ):

So the standard explicitly says that what you're asking for is
invalid.  Also, mozilla does the same as konqueror.   Seems like a
definite WONTFIX for me.

As for the thing with empty anchors, I cannot reproduce with KDE
