RFS: QA Upload -- mma - Musical Midi Accompaniment generator

Hi,

Just an upload to set QA to maintainer and bring standards, et al up to 
date.  Package should really probably be removed but we can see if 
someone adopts it first I suppose.

MMA creates midi tracks for a soloist to perform over from a user
supplied file containing chords and MMA directives.
