This is not the case in one particular scenario: one installs one of the
kernel metapackages, eg. linux-image-686, with aptitude. The image that
gets installed as a dependency gets marked as auto. On upgrade of
linux-image-686, a new package gets installed, and the old one is
removed by aptitude, because it was marked as auto.

This is probably becoming more common now that aptitude is the
recommented package management tool.
