Assuming that the above file is saved as template.tex, the
commands shall be in the following sequential order:

latex template
dvips -E -T <textwidth>,<textheight> -l1 template -o
epstopdf template.ps

You will get a warning at the end of the latex run which will have
enough instructions and also the values of textwidth and textheight.
