My first thought here was - why isn't the recommended pxlmono driver
being used? It may not be important in the light of what is said later.

The E238 has a PCL interpreter, so this shouldn't happen. Right?

The E238 also has a Postscript interpreter and it appears to work.

The last two examples only use CUPS to transport the raw data to the
printer. There is no filtering.

How is the server connected to the printer?

Is testing using unstable a possibility for you?
