Bug#545548: debian-policy package should include a pointer to http://wiki.debian.org/PolicyChangesProcess

The debian-policy change process doesn't seem to be linked from anywhere in
the debian-policy package (or the policy git repo). It would be good to have
a pointer to it, to tie this all together.
