Bug#222553: policy 11.5.3 refers to using the menu package to register docs

Document menu entry will work only with dwww and not with dhelp and
are deprecated but supported.

I suggest we add doc-base to policy and then get rid of reference
to document in '9.6. Menus' and of menu in 11.5. 3).
