I can reproduce using an up-to-date Sid with dh-make-perl 0.42

It happens because Curses::Forms needs Curses::Widgets, which is found
both in bastille and libcurses-widgets-perl, but dh-make-perl somehow
settles for the first occurrence of Curses/Widgets.pm found in bastille.

Just replace bastille for libcurses-widgets-perl in debian/control and
everything will work out fine. I believe this should be a bug on
bastille.
