To reproduce:
- load the attached doc file in LibreOffice
- go to the index and edit it
- set index depth to 3: now you should see the subchapters in the index
- save the file in the same format
- close the file
- reopen it: you should see that the index is limited to depth 1

If I save in odt format I do not see the bug.

This happens to me on LibreOffice 3.4.3 on Debian 'wheezy'.
It has has been reproduced with OOo 3.3.0 on Vista.

This bug is most likely fixed since version 4.4.0.0.alpha1+.

You can reopen the report if the problem is still apparent.
