Depends on what 'first time' means.

You have no working root fs on the first partition of your first SCSI
drive. :-) I've seen this happen when booting from a tftpimage without
'root=/dev/ram' parameter, and also at the reboot after installation
if the environment aren't set right.
The installation documentation explains both, IIRC.
