Depends: libqt5gui5, libqt5widgets5, libqt5core5a, libsqlite3-dev, ${shlibs:Depends}, ${misc:Depends}
this is usually wrong.
shlibs:Depends and misc:Depends should add them correctly.
Why do you need the runtime libsqlite3-dev development package?
I think it is wrong, shlibs will substitute it to the actually libsqliteSONAME runtime package.
(because it is linked)
