Just list all files which are under the GPL followed by the GPL header
and then all files licensed under the BSD followed by the BSD text.

Here you have a debian/copyright file with multiple copyright holders
as an example:
http://packages.debian.org/changelogs/pool/main/l/lightyears/lightyears_1.3a-3/lightyears.copyright

About the machine-interpretable format to which you link, it's indeed
not a "standard", but it isn't a bad idea to use it ;).
