I get the impression from reading [1] that you should get rid of the overly 
verbose "This package was debianized..." stuff and (re)write it like this:

Copyright YYYY-YYYY Upstream Author One
          YYYY Upstream Author Two
          ...
          YYYY-YYYY Previous Maintainer (previous Debian package maintainer)
          YYYY Current Maintainer (Debian package maintainer)
