How do I include a preseed.cfg file to d-i live?

Running the install from the boot menu work as expected, however, doing a live install doesn't. The preseed/late_command is not executed.

The preseed file is here: config/debian-installer/preseed.cfg

I have this working in Jessie, but wheezy is giving me grief.
