Rather than requesting that, why don't you use live-helper to create
such an image-file and publish it yourself?

The beauty of the live-helper package is that it makes that very easy
to do

First install a current version of the packages. If you have, or can
get, root-access on a lenny or sid system (testing or unstable) then
it is as simple as

# apt-get install live-helper

To install on etch, read http://live.debian.net/manual/html/ch02s02.html

Then, you only issue (as root)

lh_config -p "my-packages" -b iso
echo "#include <gnome>" > config/chroot_local-packageslists/my-packages
echo "gnome-orca" >>      config/chroot_local-packageslists/my-packages
lh_build

and live-helper will create the image-file for you.

Perhaps debian-live could have a webbpage with a table of links to 

a) contributed images
b) the scripts used to build these images
