Operation success but patient died. That is lh_build continued past the
original problem, posted three times a note that the system must be
rebooted before using loop, then a message to ignore the previous
warnings, then used loop and exited. At this point a directory binary
had been created but no binary.img in the base directory. I found a
file binary.img in chroot and dd it to the memory stick. Boot from the
memory stick failed - no operating system found.
On rebooting from the hard drive, hdb5 had to be repaired. (Debian
lenny is on hdb, hda used to be windoz but has been converted to a large
ext2 partition.)
