Bug#697161: [new check] check for patches that apply changes twice

dpkg-source on squeeze and earlier errors out when one of the
dpkg-source 3.0 (quilt) patches modifies one file twice. With
dpkg-source on wheezy and later this becomes a warning instead. I've
encountered this issue while unpacking source packages from the Debian
derivatives census, an example is below. This should be an autoreject at
least until squeeze is EOL.
