No.  That license specifically covers only versions for the "Windows PC"
and "Macintosh desktop operating systems" (see 1(c)).

There are many other problems with it, such as most of the clauses in
section 2(b), and sections 4, 7, and 8.

Also, exactly what "Licensee Product" do you have in mind?  This
agreement must be signed, and Debian won't sign such agreements.
So that would make you the Licensee and Debian a redistributor.
(Note that 2(b)(viii) puts restrictions on distributors as well.)

I'm surprised you ever thought this license would allow the software
to be packaged for non-free.  Did you give the right url?

I doubt there's a template for this.  I think the best way to ask is
simply to describe what you want to do, explain that their standard
license won't work (otherwise they'll simply reply with a pointer to
that license), and ask for permission.  When describing what you want
to do, pay special attention to exactly how, in what format, and to
whom their product will be distributed.
