it seems to be the same modem that is running in my notebook.
the pci-id is 1039:7013
the output of lspci on my (SuSE 8.2) system is:

(rev a0)
....
It is a winmodem. You can download the driver from SmartLink in Isreal:
http://www.smlink.com.
The driver is not open source though.
