Bug#898825: linux-image-4.16.0-1-amd64: KMS/graphics not working on Lenovo P50 with 4.16

When booting 4.16 on my Lenovo P50, the plymouth splash screen never
appears, and I'm left starting at the text that appeared before it tried
to start drawing on the screen.

When booting with plymouth disabled, the system behaves normally until
Xorg starts, where again I'm left starting at the prior screen contents.

In both cases I can eventually reboot with Ctrl+Alt+Del.

In case it's relevant... this particular laptop has a muxless hybrid
graphics configuration.  Plymouth normally happily displays on both the
internal display (via the Intel GPU) and the external one (via the
NVIDIA GPU) without any special configuration. Xorg requires me to run
'xrandr --setprovideroutputsource 0x49 0xd5' to make the NVIDIA outputs
available to Xorg using the Intel GPU, but that doesn't happen until I
log in, and the problem manifests itself when GDM launches Xorg using
only the Intel GPU.

4.15 works fine (well, as good as it can with this POS NVIDIA card...)
