I had the same problem. Fortunately I know why that happened. Due to
some reason, a grub(2) upgrade and a disk replace caused some wrong
entry in /boot/grub/device.map. To fix the problem, I run the following
command manually
# cat /boot/grub/device.map
# sudo grub-mkdevicemap
# cat /boot/grub/device.map
and a new reboot works like a charm.
