Thanks for the quick feedback it is indeed flash-kernel hook that's making
the trouble.

add such a line on top of
/usr/share/initramfs-tools/hooks/flash_kernel_set_root file with:
set -x

rerun and please post output of:
 sh /usr/sbin/mkinitramfs -o /tmp/foo
