This happens when the device is rapidly assigned and then de-assigned to
a VM. This is happening on my test system because libvirt decides the
devices I'm choosing are unsuitable, but not before assigning them (very
weird behaviour).
There is a workaround which is to use kernel parameter
'intel_iommu=on,strict' rather than simply 'intel_iommu=on', but this
can have a significant performance impact.
