Bug#674907:



I have the same issue running wheezy with a recent custom compiled kernel.
I also used to have the issue under squeeze ...

Until "recently" I just worked around the issue by forcing ntp resync
even for large offset but recently I got an application that can't
tolerate even the small period of time between the unsync and the ntp
resync ...

It doesn't happen at regular intervals, more like randomly and yes, it
cause the offset on dom0 and all domUs running there.
