This seems to be relevant and contains a patch:

This is how I reproduce it:
System just booted, chromium NOT running:
root@viaza:~# lvcreate -L 1G -n foo viaza
Logical volume "foo" created
root@viaza:~# lvremove /dev/viaza/foo
Do you really want to remove active logical volume foo? [y/n]: y
Logical volume "foo" successfully removed
After starting chromium:
root@viaza:~# lvcreate -L 1G -n foo viaza
/dev/mapper/viaza-foo: open failed: No such file or directory
/dev/viaza/foo: not found: device not cleared
Aborting. Failed to wipe start of new LV.
LV viaza/foo in use: not deactivating
Unable to deactivate failed new LV. Manual intervention required.
After closing chromium:
root@viaza:~# lvcreate -L 1G -n bar viaza
Logical volume "bar" created
After reopening chromium:
root@viaza:~# lvcreate -L 1G -n baz viaza
/dev/mapper/viaza-baz: open failed: No such file or directory
/dev/viaza/baz: not found: device not cleared

After closing chromium:
root@viaza:~# lvcreate -L 1G -n wibble viaza
Logical volume "wibble" created
...and so on. Nothing shows up in dmesg: the version you see attached is taken
just after running the sequence of commands above.
Chromium is running as a normal user, which means that whatever it is doing can
make root unable[1] to create LVM volumes, which sounds to me like a security
issue as well.
