This _might_ have been fixed by v2.6.32-rc1~697^2 (fsync: wait for
data writeout completion before calling ->fsync, 2009-09-03).  If so,
the fix doesn't seem to be part of lenny or v2.6.27.y yet.  If you are
still on lenny and can provoke this at will (unlikely, I know), please
try a lenny-bpo kernel so we can see if that guess is plausible.

That's 2.6.26-17lenny1, I think.

syslog and kern.log would have said something like the following, near
the start of bootup, fwiw:
